Job summary

Hamilton Health Center is seeking an LPN Care Coordinator to support patient-centered care at the Newport Family Care location. This role promotes collaboration among the patient-centered care team to ensure quality care, increase preventive care utilization, and reduce ER visits and readmissions.

The LPN Care Coordinator provides care coordination for patients, including those with higher-risk or complex needs.

Qualifications

• High school diploma or GED.
• Graduate of an accredited Licensed Practical Nursing program.
• Current license to practice as a Licensed Practical Nurse in Pennsylvania.
• At least two years of experience working with patients and families in community settings.
• Prior experience with care coordination or care management in a clinical setting.
• Proficiency in basic data entry and computer skills.

Responsibilities

• Provide care coordination services involving case management and patient advocacy.
• Co-manage higher-risk patients through assessment and care coordination activities.
• Maintain ongoing interaction with patients and families to support adherence to treatment plans.
• Educate patients and families regarding the Patient-Centered Care Model and care needs.
• Serve as a liaison for patients regarding medical and dental benefits.
• Coordinate with providers and the care team to support continuity and quality of care.
• Monitor care delivery by documenting progress and identifying barriers to care.
• Conduct home and hospital visits as needed for care coordination.
• Ensure HIPAA compliance and confidentiality of information.
• Collect and utilize data to support service improvement and monthly reporting.
• Perform additional job-related duties as assigned.
