Job summary

Amedisys is seeking a detail‑oriented Intake Coordinator for our Windy Hill Beach, SC area. This role schedules home visits for field staff and supports patient care coordination under the Clinical Manager to optimize resource use and service delivery.

You will handle scheduling with patients and clinicians, manage reports, and assist with transfer of patients between care centers. RN/LPN licensure may be required for field duties; strong communication is essential.

Qualifications

• Current, unencumbered license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse or Registered Nurse specific to SC.
• High School diploma or GED equivalent.
• One (1+) year of administrative experience in a health care environment.

Responsibilities

• Schedules patient visits and Oasis visits timely.
• Prepares weekly clinician schedules and monitors capacity.
• Optimizes geography to match skills with required care and maximize efficiency.
• Handles calls from patients and field staff about scheduling.
• Supports transfer of patients between care centers or Hospice services.
• May provide direct patient care in emergencies if licensed and needed.
