Qualifications

Responsibilities

• Schedules patient assessment visits including Oasis visits to ensure timely performance.
• Prepares weekly clinician schedules and monitors capacity to meet productivity standards.
• Assigns clinicians efficiently by geography and skill to maximize capacity and minimize travel.
• Ensures missed visits are rescheduled or escalated to Clinical Manager for resolution.
• Manages calls from patients and field staff regarding scheduling issues and referrals to Clinical Manager.
• Monitors hospitalized patients and ensures appropriate follow-up by the team.
