Job summary

CenterWell Home Health in Florida is seeking a Patient Services Coordinator-LPN to schedule visits and communicate with field staff, patients, and physicians to maintain care coordination and continuity of care. The role also supports day-to-day office and staff management.

The position requires an active nursing license (LPN or RN depending on state), at least one year of home health experience, strong communication, organization, and the ability to manage multiple tasks.

Qualifications

• Must hold an active LPN or RN license in the practicing state.
• At least 1 year of home health experience.
• QI or packet review experience preferred.
• Coding certification preferred.
• Valid state driver’s license and auto insurance required.
• Active license in the state of employment if applicable.
• Excellent communication, organization, and multitasking skills.

Responsibilities

• Schedule visits and coordinate with field staff to ensure timely patient care.
• Communicate with patients, physicians, and staff to maintain care coordination.
• Assist with day-to-day office and staff management.
• Process and review scheduling and log data; generate reports.
• Support on-call rotation and field support as needed.
