Job summary

AMEDISYS HOME HEALTH is seeking an On-Site LPN Patient Service Coordinator - Clinical to schedule home visits and coordinate patient care under the Clinical Manager. This role requires an active LPN license and is based in an on-site office in North Carolina.

Qualifications

• Current, unencumbered license to practice as an LPN/LVN or RN for the state of employment.
• High School diploma or GED or equivalent.
• At least 1 year of administrative experience in a health care setting.

Responsibilities

• Schedules home visits for field staff and coordinates patient care.
• Prepares weekly clinician schedules and reviews capacity to meet productivity standards.
• Manages scheduling communications with patients, field staff, and Clinical Manager.
• Monitors visits, ensures visits are completed as ordered, and refers issues as needed.
• Assists with referrals, intake, and transfer of patients between care centers.
