Job summary

Qualifications

• Current, unencumbered LPN/LVN or RN license for the state of employment.
• High School diploma or GED.
• One year (1+) of administrative experience in a health care environment.

Responsibilities

• Schedules patient assessment visits and Oasis visits in a timely manner.
• Prepares weekly clinician schedules and reviews capacity to meet productivity standards.
• Allocates clinicians by geography to maximize efficiency and match skill to care.
• Ensures missed visits are rescheduled or referred to the Clinical Manager.
• Communicates with patients and field staff to coordinate scheduling of services.
• Monitors patient status and ensures follow-up as needed.
• Supports transfer of patients between care centers and Hospice services.
