Job summary

Bethesda Health Group is seeking a Licensed Practical Nurse to provide basic nursing care to geriatric residents, assess changes, and communicate with the IDT. The role includes documenting assessments and updating care plans, with a focus on safe medication administration and timely physician communication.

The LPN must hold a current Missouri license, CPR/AED and IV certification, and be prepared for lifting and transferring patients with assistance.

Qualifications

• Education from an accredited school of nursing resulting in LPN
• Current LPN license in the state
• CPR and AED certification required or to be obtained within six months
• IV certification required or to be obtained within six months
• Long-term care experience preferred
• Ability to assess basic nursing care needs of geriatric residents
• Excellent communication skills with residents, visitors and staff
• Time management and organizational skills
• Ability to stand and walk for long periods
• Ability to lift heavy objects with assistance when necessary
• Ability to transfer and move patients weighing 200+ lbs with assistance
• Ability to push/pull equipment over 40 lbs with assistance
• Manual dexterity and coordination for patient care
• Visual acuity and ability to distinguish color
• Ability to document electronically and on paper

Responsibilities

• Provide basic nursing care and assess residents on admission and ongoing
• Communicate significant changes to interdisciplinary team (IDT)
• Document assessments accurately and timely
• Participate in care planning and update plans as interventions change
• Administer medications and treatments safely
• Perform IV therapy only if IV certified
• Notify physicians timely using SBARR method
• Monitor care plan delivery and adjust as needed
• Assist with ADLs and respond to call lights
• Contribute to problem-solving using QAPI process
