Bethesda Health Group in Missouri seeks an experienced Licensed Practical Nurse to provide basic nursing care and resident assessments. You will collaborate with the Interdisciplinary Team to implement the care plan, document care, and communicate changes to residents and families.

The role includes medication administration, IV care if certified, and ongoing education. Strong communication, time management, and mobility are essential to support geriatric residents and maintain high-quality care

Qualifikationen

• Education from an accredited school of nursing that results in becoming a Licensed Practical Nurse.
• Current LPN license in the state you will be working in.
• CPR and AED certification required, or willing to obtain within six months of employment.
• IV certification required, or willing to obtain within six months of employment.
• Long-term care experience preferred.
• Must be able to assess the basic nursing care needs of the geriatric resident.
• Excellent communication skills to interact with residents, visitors and staff.
• Time management and organizational skills required.
• Ability to stand and walk for long periods of time.
• Ability to lift heavy objects using proper lifting techniques and with assistance when necessary.
• Ability to transfer and move patients who weigh 200+lbs using proper lifting techniques and with assistance.
• Ability to push and pull equipment weighing in excess of 40lbs using proper techniques and with assistance.
• Manual dexterity, coordination and skillful use of hands when working with patients and equipment.
• Visual acuity with the ability to distinguish color.
• Ability to physically perform necessary documentation electronically and in writing.

Aufgaben

• Provides basic nursing care by assessing the resident on admission and on an ongoing basis.
• Communicates significant changes to the appropriate IDT member and residents and families.
• Documents all assessments accurately.
• Participates in the care planning process and updates the plan of care as needed.
• Implements nursing interventions safely and timely, including medication administration.
• Administers IV fluids only if IV certified.
• Notifies physician timely using the SBARR communication tool.
• Assists with crisis situations calmly and professionally.
• Monitors care plan delivery and addresses issues using QAPI processes.
• Answers call lights and assists with residents' ADLs.
• Completes twelve hours of nursing-related continuing education annually.
