Complete Care Management in Windsor, CT is seeking an LPN Licensed Practical Nurse for weekend, 3rd shift coverage. PRN also considered for experienced candidates.

You will support residents recently discharged from hospital and those requiring long-term care with therapy and skilled nursing services. Key duties include medication administration, wound care, monitoring health status, and collaboration with physicians and therapists.

Qualifications

• LPN license is required and must be valid for the state.
• Experience with medication administration and wound care.
• Ability to monitor patient status and communicate changes.

Here's What You'll Do for Our Residents:

Administer Medications: Give oral, topical, IV, injectable medications

Wound/Skin Care: Perform wound care/prevention, manage pain, apply dressings

Monitoring: Observe physical and mental health status- track changes & analyze data

Clinical Procedure: Provide catheter/ostomy care, gather specimens etc.

Care Planning: Assist in executing individualized care

Collaboration: Strategize care with physicians, therapists, social workers & others

Family & Resident Education: Explain health status, changes and expectations for care

Documentation: Keep a detailed record of health status, all care, progress & interactions

Team Oversight: Oversee & delegate CNA's ensuring quality care/teamwork
