Complete Care, Inc. is seeking a Licensed Practical Nurse (LPN) for Direct Care roles in Hartford, CT. Positions include Full Time 2nd Shift, Part Time 3rd Shift, and PRN with experience.

• Licensed LPN in Connecticut
• Ability to pass background check, drug screen, and reference checks

Responsibilities

• Administer medications (oral, topical, IV, injectable)
• Perform wound care and manage dressings
• Monitor health status and analyze data
• Provide catheter/ostomy care and collect specimens
• Assist in executing individualized care plans
• Collaborate with physicians, therapists, social workers
• Educate residents and families on health status and care expectations
• Maintain detailed health and care documentation
• Oversee CNAs to ensure quality care and teamwork

Here's What You'll Do for Our Residents:

Administer Medications: Give oral, topical, IV, injectable medications

Wound/Skin Care: Perform wound care/prevention, manage pain, apply dressings

Monitoring: Observe physical and mental health status- track changes & analyze data

Clinical Procedure: Provide catheter/ostomy care, gather specimens etc.

Care Planning: Assist in executing individualized care

Collaboration: Strategize care with physicians, therapists, social workers & others

Family & Resident Education: Explain health status, changes and expectations for care

Documentation: Keep a detailed record of health status, all care, progress & interactions

Team Oversight: Oversee & delegate CNA's ensuring quality care/teamwork
