Job summary

Complete Care Management in Connecticut is seeking a Part-Time LPN for 2nd shift options. You will administer medications, assist with wound care, monitor residents, and collaborate with physicians and other caregivers.

Qualifications

• Licensed to work within CT as an LPN.
• Able to pass background check, drug screen, references, and physical evaluation.

Responsibilities

• Administer medications (oral, topical, IV, injections).
• Provide wound care and prevention; manage pain; apply dressings.
• Monitor health status and track changes.
• Perform catheter/ostomy care and specimen collection.
• Assist in executing individualized care plans.
• Collaborate with physicians, therapists, social workers and others.
• Educate residents and families about health status and care expectations.
• Maintain detailed health records and care progress.
• Oversee and delegate tasks to CNAs to ensure quality care.

Here’s What You’ll Do for Our Residents:

Administer Medications: Give oral, topical, IV, injectable medications

Wound/Skin Care: Perform wound care/prevention, manage pain, apply dressings

Monitoring: Observe physical and mental health status- track changes & analyze data

Clinical Procedure: Provide catheter/ostomy care, gather specimens etc.

Care Planning: Assist in executing individualized care

Collaboration: Strategize care with physicians, therapists, social workers & others

Family & Resident Education: Explain health status, changes and expectations for care

Documentation: Keep a detailed record of health status, all care, progress & interactions

Team Oversight: Oversee & delegate CNA's ensuring quality care/teamwork
