Job summary

Complete Care Management is hiring a Licensed Practical Nurse (LPN) to provide direct care to residents recently discharged or in long-term care. This position offers multiple shifts including full-time 2nd shift, part-time 3rd shift, and weekends with differential pay and overtime eligibility.

As an LPN, you will administer meds, monitor health status, assist in care planning, and collaborate with physicians, therapists, and family members to ensure high-quality resident care.

Qualifications

• Licensed to work within this state as a LPN.
• Able to pass background check, drug screen, professional references check & physical evaluation
• Professional & Clinically Skilled with leadership qualities

Responsibilities

• Administer Medications: Give oral, topical, IV, injectable medications
• Wound/Skin Care: Perform wound care/prevention, manage pain, apply dressings
• Monitoring: Observe physical and mental health status- track changes & analyze data
• Clinical Procedure: Provide catheter/ostomy care, gather specimens etc.
• Care Planning: Assist in executing individualized care
• Collaboration: Strategize care with physicians, therapists, social workers & others
• Family & Resident Education: Explain health status, changes and expectations for care
• Documentation: Keep a detailed record of health status, all care, progress & interactions
• Team Oversight: Oversee & delegate CNA’s ensuring quality care/teamwork

• PRN Accepted with Experience

• 2nd Shift & Weekend Differential Pay

• Overtime Pay after 8hrs
