Job summary

Heritage | Complete Care at Heritage in Baltimore, MD is seeking an LPN Licensed Practical Nurse to care for residents recently discharged from hospital or needing long-term care. You will administer medications, perform wound care, monitor health status and collaborate with physicians and therapists to ensure quality care.

We offer full- and part-time options, sign-on bonuses of $6,000 for FT and $3,250 for PT, daily pay option, and a benefits package including medical, dental, vision, life,

Qualifications

• Must be licensed to practice in Maryland.
• Pass background check, drug screen, and employment references.
• Professional & clinically skilled with leadership qualities.

Responsibilities

• Administer medications (oral, topical, IV, injections).
• Wound care and prevention; manage pain and dressings.
• Monitor health status and track changes.
• Provide catheter/ostomy care and collect samples.
• Assist in executing individualized care plans.

Here’s What You’ll Do for Our Residents:

Administer Medications: Give oral, topical, IV, injectable medications

Wound/Skin Care: Perform wound care/prevention, manage pain, apply dressings

Monitoring: Observe physical and mental health status- track changes & analyze data

Clinical Procedure: Provide catheter/ostomy care, gather specimens etc.

Care Planning: Assist in executing individualized care

Collaboration: Strategize care with physicians, therapists, social workers & others

Family & Resident Education: Explain health status, changes and expectations for care

Documentation: Keep a detailed record of health status, all care, progress & interactions

Team Oversight: Oversee & delegate CNA’s ensuring quality care/teamwork
