Job summary

Complete Care Management is seeking an LPN licensed Practical Nurse in Delaware to support Direct Care services for residents recently discharged from hospital or in long‑term care. The role involves clinical tasks, coordination with care teams, and education for families and residents.

The position requires weekend 3rd shift availability, strong nursing skills, and the ability to work collaboratively in a busy care environment. Rewards include growth opportunities and generous benefit options.

Qualifications

• Licensed to practice as an LPN in Delaware.
• Ability to pass background check, drug screen, references, and physical.
• Professional and clinically skilled with leadership qualities.

Responsibilities

• Administer medications: oral, topical, IV, injectable meds.
• Wound care, prevention, pain management and dressings.
• Monitor health status and analyze data for changes.
• Provide catheter/ostomy care and gather specimens.
• Assist in executing individualized care plans.
• Collaborate with physicians, therapists and social workers.
• Educate residents and families on health status and care expectations.
• Maintain detailed documentation of health status and care.
• Oversee and delegate CNA tasks to ensure quality care

Here’s What You’ll Do for Our Residents:

Administer Medications: Give oral, topical, IV, injectable medications

Wound/Skin Care: Perform wound care/prevention, manage pain, apply dressings

Monitoring: Observe physical and mental health status- track changes & analyze data

Clinical Procedure: Provide catheter/ostomy care, gather specimens etc.

Care Planning: Assist in executing individualized care

Collaboration: Strategize care with physicians, therapists, social workers & others

Family & Resident Education: Explain health status, changes and expectations for care

Documentation: Keep a detailed record of health status, all care, progress & interactions

Team Oversight: Oversee & delegate CNA’s ensuring quality care/teamwork
