Complete Care Management is seeking an LPN Licensed Practical Nurse for direct care in Delaware. You will administer medications and provide wound and skin care, monitoring residents closely and coordinating with the care team.

Qualifications

• Must be licensed to work within the State of Delaware as an LPN.

Responsibilities

• Administer Medications: oral, topical, IV, injectable medications
• Wound/Skin Care: wound care and dressings
• Monitoring: observe health status and track changes
• Clinical Procedure: catheter/ostomy care, gather specimens
• Care Planning: assist in executing individualized care
• Collaboration: strategize care with physicians, therapists, social workers & others
• Family & Resident Education: explain health status, changes and expectations for care
• Documentation: keep a detailed record of health status, all care, progress & interactions
• Team Oversight: oversee & delegate CNAs ensuring quality care/teamwork
