Autumn Lane Health & Rehab is seeking a Licensed Practical Nurse to provide high-quality, person-centered care and work closely with RNs, CNAs and the interdisciplinary team to support residents.

Qualifications

• Active Licensed Practical Nurse (LPN) license (State of Georgia or eligible).
• Experience in long-term care preferred but not required.
• Ability to deliver high‑quality, person‑centered care.

Responsibilities

• Administer medications and treatments as prescribed
• Monitor residents' conditions and report changes to the care team
• Provide direct nursing care in accordance with care plans
• Supervise and support CNAs in delivering quality care
• Maintain accurate and timely documentation
• Ensure compliance with clinical policies and regulatory standards
• Communicate effectively with residents, families, and team members

Your Impact as an LPN

As a Licensed Practical Nurse, you play a critical role in delivering high‑quality clinical care and ensuring the well‑being of our patients. You will work closely with RNs, CNAs, and the interdisciplinary team to provide compassionate, person‑centered care.
