Job summary

Autumn Lane Health & Rehab is seeking an LPN to deliver high-quality clinical care in a supportive team. You will work with RNs and CNAs to provide compassionate, person-centered nursing, administer medications, monitor residents, and document care plans.

The position offers pay ranging from $26.25 to $29.50 per hour with evenings, nights and weekends shift differentials, plus weekly pay and a range of benefits in a non-profit, mission-driven setting.

Your Impact as an LPN

As a Licensed Practical Nurse, you play a critical role in delivering high-quality clinical care and ensuring the well-being of our patients. You will work closely with RNs, CNAs, and the interdisciplinary team to provide compassionate, person-centered care.

Key Responsibilities:

• Administer medications and treatments as prescribed
• Monitor residents' conditions and report changes to the care team
• Provide direct nursing care in accordance with care plans
• Supervise and support CNAs in delivering quality care
• Maintain accurate and timely documentation
• Ensure compliance with all clinical policies and regulatory standards
• Communicate effectively with residents, families, and team members

What You Bring

• Active Licensed Practical Nurse (LPN) license (State of Georgia or eligible)
• A passion for delivering compassionate, high-quality care
• Strong clinical judgment and critical thinking skills
• Excellent communication and teamwork abilities
• Reliability and a commitment to excellence
• Prior long-term care experience preferred, but not required
