Job summary

Autumn Lane Health & Rehab is seeking a Licensed Practical Nurse to deliver high-quality clinical care and ensure the wellbeing of residents. You will work closely with RNs, CNAs, and the interdisciplinary team to provide compassionate, person-centered care.

Responsibilities include administering medications, monitoring conditions, and maintaining documentation in a supportive, mission-driven nonprofit environment.

Qualifications

• Active Licensed Practical Nurse (LPN) license (State of Georgia or eligible).
• A passion for delivering compassionate, high‑quality care.
• Strong clinical judgment and critical thinking skills.
• Excellent communication and teamwork abilities.
• Reliability and a commitment to excellence.
• Prior long‑term care experience preferred, but not required.

• Administer medications and treatments as prescribed
• Monitor residents’ conditions and report changes to the care team
• Provide direct nursing care in accordance with care plans
• Supervise and support CNAs in delivering quality care
• Maintain accurate and timely documentation
