Job summary

Greene Point Health & Rehab in Georgia is seeking a dedicated Licensed Practical Nurse (LPN) to join our mission-driven team. You will provide high-quality, patient-centered care alongside RNs, CNAs, and the interdisciplinary team, delivering medications, monitoring conditions, and maintaining accurate documentation.

Qualifications

• Active LPN license (Georgia or eligible) required.
• Strong clinical judgment and critical thinking.

• Prior long-term care experience preferred, but not required.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor residents’ conditions and report changes to the care team.
• Provide direct nursing care in accordance with care plans.
• Supervise and support CNAs in delivering quality care.
• Maintain accurate and timely documentation.
• Ensure compliance with all clinical policies and regulatory standards.
• Communicate effectively with residents, families, and team members.

Your Impact as an LPN

As a Licensed Practical Nurse, you play a critical role in delivering high-quality clinical care and ensuring the well-being of our patients. You will work closely with RNs, CNAs, and the interdisciplinary team to provide compassionate, person-centered care.
