Job summary

CHSGa seeks a Licensed Practical Nurse to deliver high-quality, patient-centered care within a nonprofit care setting. You will work closely with RNs, CNAs, and the interdisciplinary team to implement care plans and support residents and families.

In this role you’ll administer treatments, monitor conditions, document accurately, and ensure regulatory compliance while upholding CHSGa’s compassionate care standards and mission across Georgia.

Qualifications

• Active LPN license in GA or eligibility.
• Commitment to high-quality patient care.
• Strong communication with the care team, residents, and families.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor residents’ conditions and report changes to the care team.
• Provide direct nursing care in accordance with care plans.
• Supervise and support CNAs in delivering quality care.
• Maintain accurate and timely documentation.
• Ensure compliance with all clinical policies and regulatory standards.
• Communicate effectively with residents, families, and team members.

Your Impact as an LPN

As a Licensed Practical Nurse, you play a critical role in delivering high-quality clinical care and ensuring the well-being of our patients. You will work closely with RNs, CNAs, and the interdisciplinary team to provide compassionate, person-centered care.

What You Bring

• Prior long-term care experience preferred, but not required
