Job summary

Monarch Healthcare Management is seeking an LPN Care Coordinator to support nursing leadership in managing a specific unit. You will assist with care planning, data collection, and implementing the nursing process under RN supervision, ensuring resident needs are met and documented accurately.

You will participate in care conferences, monitor conditions, and help develop comprehensive plans while promoting a safe, positive, and inclusive workplace culture.

Qualifications

• Licensure and Certification: Must hold a current MN LPN license and CPR certification.
• Communication: strong verbal and written communication; able to work under RN supervision.
• Preferred Experience: healthcare setting, skilled nursing or assisted living experience preferred.
• Core Skills: multitasking, adaptability, active listening, teamwork.

Responsibilities

• Assist with care planning and data collection for residents.
• Contribute to MDS/RAPS assessments and update care plans.
• Collaborate with interdisciplinary team to meet resident needs.
