Monarch Healthcare Management is seeking an LPN Care Coordinator to support nursing leadership, manage care planning, and ensure high-quality resident care across a Minnesota facility.

The role involves data collection, assessment participation, and coordination with RNs and nursing staff to meet resident needs while maintaining documentation, regulatory compliance, and a patient-centered care approach.

Qualifications

• Must hold a current LPN license in Minnesota and CPR/emergency care certification.
• Strong written and verbal communication skills.
• Healthcare setting experience preferred (skilled nursing or assisted living) with knowledge of Medicare.
• Ability to multitask, adapt to change, and work in a fast-paced, diverse environment.

Responsibilities

• Assist with care planning, data collection, and updating care plans under RN supervision.
• Monitor resident conditions, report changes, and participate in care conferences.
• Support staffing needs and ensure safe, competent care delivery.
• Maintain accurate documentation and comply with nursing policies and regulatory requirements.
