The role involves data collection for assessments (MDS/RAPS), developing care plans, and collaborating with the interdisciplinary team to meet resident needs while maintaining accurate documentation and compliance with policies and regulations.

Qualifikationen

• Must hold a current LPN license in Minnesota and CPR/emergency care certification.
• Ability to communicate effectively in writing and verbally.
• Experience in healthcare settings such as skilled nursing or assisted living is preferred.
• Strong multitasking, adaptability, and collaboration skills.
