Job summary

Monarch Healthcare Management is seeking an LPN Care Coordinator to support nursing leadership in planning and managing a specific unit. The role involves data collection, care plan implementation, and collaboration with RNs and the interdisciplinary team to meet resident needs.

The position requires an active Minnesota LPN license, CPR, and a commitment to accurate documentation, regulatory compliance, and quality improvement. A positive, inclusive workplace culture is emphasized.

Qualifications

• Must hold a current LPN license in Minnesota and CPR/emergency care certification.
• Must be able to effectively communicate in verbal and written forms and perform all essential job functions with or without reasonable accommodation.
• Experience in healthcare settings like skilled nursing or assisted living is preferred, along with familiarity with Medicare coverage and eligibility.
• Requires strong multitasking, adaptability, emotional intelligence, active listening, and the ability to work effectively in fast-paced, diverse, and evolving environments.

Responsibilities

• Assist with care planning, data collection, and implementation of the nursing process.
• Monitor resident conditions, report changes to RNs, participate in care conferences, and help ensure care plans reflect current physician orders.
• Provide nursing care under RN supervision, support and direct LPNs and nursing assistants, assist with staffing needs.
• Maintain accurate documentation, communicate care needs effectively, adhere to nursing policies and regulatory requirements, and support quality improvement initiatives.
