Monarch Healthcare Management is seeking an LPN Care Coordinator to support the Director of Nursing and RN Manager in leading a nursing unit. The role involves coordinating resident care plans, data collection, and ensuring quality outcomes through effective resource use.

You will assist with developing and implementing care plans, communicating with the team and families, and helping guide the nursing process under nurse leadership.

Qualifications

• Must hold active LPN license in Minnesota.
• Must have or seek CPR/certification for health care providers.
• Must be able to communicate clearly, in writing and verbally.

Responsibilities

• Assist Director of Nursing and RN Manager in nursing unit leadership.
• Coordinate resident care plans and data collection under supervision.
• Support care delivery and evaluation of resources for quality outcomes.
• Help implement and revise care plans with the nursing team.
