Job summary

Monarch Healthcare Management is seeking an LPN Care Coordinator to support nursing leadership in a specific unit. You will assist with care planning, data collection, and ensuring care plans reflect physician orders, while collaborating with the interdisciplinary team to meet resident needs.

You will monitor resident conditions, report changes to RNs, participate in care conferences, and provide nursing care under RN supervision.

Qualifications

• Must hold a current LPN license in Minnesota and CPR/emergency care certification; must meet MN DHS background check.
• Strong communication and ability to perform essential job functions with or without accommodation.
• Preferred experience in healthcare settings (skilled nursing or assisted living) and familiarity with Medicare coverage and eligibility.
• Core skills: multitasking, adaptability, emotional intelligence, active listening, and teamwork in a fast-paced environment.

Responsibilities

• Assist with care planning, resource evaluation, and nursing process implementation.
• Collect resident data and contribute to MDS/RAPS assessments; develop and update care plans.
• Collaborate with interdisciplinary team to meet resident needs; monitor conditions and report changes to RNs.
• Provide nursing care under RN supervision and support LPNs and nursing assistants.
• Assist with staffing needs; ensure safe, competent care delivery and maintain documentation.
• Promote a positive, inclusive, customer-focused workplace culture and comply with policies.
