Monarch Healthcare Management in Minnesota is seeking an LPN Care Coordinator to support the Director of Nursing, manage care planning, and assist with data collection within a nursing unit. You will collaborate with RNs and the interdisciplinary team to ensure resident needs are met and nursing care aligns with physician orders.

The role includes supervising LPNs and nursing assistants, assisting with staffing, and maintaining compliant documentation in a fast-paced, evolving environment.

Qualifications

• Must hold a current Minnesota LPN license and CPR/emergency care certification.
• Ability to communicate effectively in both verbal and written forms.
• Experience in healthcare settings is preferred, familiarity with Medicare coverage is a plus.

Responsibilities

• Assist with care planning, data collection, and implementation of the nursing process.
• Monitor resident conditions and report changes to RNs.
• Support and direct LPNs and nursing assistants; assist with staffing needs.
• Maintain accurate documentation and adhere to nursing policies and regulatory requirements.
• Participate in quality improvement initiatives and ongoing licensure maintenance.
