Job summary

Monarch Healthcare Management seeks a dedicated LPN Care Coordinator to support nursing leadership in managing a specific unit. You will help evaluate resources to ensure quality outcomes of care and assist in the care planning process under Nursing Directors and RN leadership.

Responsibilities include aligning care plans with resident needs, collecting data, and implementing plans within a defined delivery system to maintain high-quality care across Monarch's facilities.

Qualifications

• Must have a current LPN registration in the state of Minnesota.
• Must have successfully completed or seek certification in Cardiopulmonary Resuscitation and emergency care for the health care provider.
• Must possess the ability to communicate verbally and in writing when coordinating the care of the residents.

Responsibilities

• Assist the Director of Nursing/Assistant Director of Nursing and RN Nurse Manager in managing a nursing unit.
• Help evaluate available resources to ensure quality outcomes of care.
• Assist with the development and implementation of care plans and revision of plans as needed.
