Job summary

Monarch Healthcare Management is seeking an LPN Care Coordinator to support the Nurse Manager and Director of Nursing in Minnesota. You will help evaluate resources, participate in care planning, and oversee the delivery of care to residents within our defined system.

You will coordinate care plans, collect data, and assist with implementation and revision to ensure quality outcomes. A Minnesota LPN license and CPR certification are required, and strong communication skills are essential.

Qualifications

• Must have strong verbal and written communication when coordinating care of residents.

Responsibilities

• Assist Director of Nursing and Assistant Director of Nursing and RN Nurse Manager in leadership of a nursing unit.
• Assist in evaluation of available resources to assure quality outcomes of care.
• Assist with systematic data collection and development of care plans.
• Implement and revise care plans as needed.
• Coordinate the management of residents' care through the defined care delivery system.
