Job summary

Traditional Home Health Care is seeking a compassionate Licensed Practical Nurse (LPN) to support pediatric and adult patients in home settings around Tobyhanna, PA. This role offers flexible hours, weekly pay, and the opportunity to deliver one-on-one care with a strong focus on safety and quality outcomes.

You will administer medications, implement and document care plans, perform patient assessments, and train family members while coordinating with a Clinical Manager and physician to ensure

Qualifications

• Active LPN license.
• Experience in home health care or pediatric/adult skilled nursing preferred.
• Ability to provide one-on-one patient care in home settings.
• Strong communication and documentation skills.

Responsibilities

• One to One patient care in home/school environment.
• Implement and document the plan of care to ensure quality and continuity of care for patient.
• Administration of prescribed medication, treatments and therapies.
• Patient assessments; coordination of care.
• Reports any changes in the client condition to Clinical Manager and physician.
• Health promotion, teaching, and training of family members.

Rate: $28-$32 per hour
