Job summary

Traditional Home Pediatric/Adult Skilled Division in Montrose, PA is seeking a compassionate Licensed Practical Nurse (LPN) to join our home health care team. This role offers Sunday–Saturday overnight shifts with a pay rate of $28–$32 per hour, plus benefits and flexible scheduling.

You will provide one-to-one patient care in the homes of pediatric and adult patients, and help with planning and implementing care plans.

Qualifications

• LPN license in good standing and currently licensed.
• Experience in home health care is a plus.
• Availability for overnight shifts and weekend coverage.

Responsibilities

• Provide one-to-one patient care in home or school settings.
• Implement and document the care plan to ensure quality and continuity.
• Administer prescribed medications, treatments and therapies.
• Perform patient assessments and coordinate care with clinicians.
• Report changes to the Clinical Manager and physician.
• Educate and train family members on health maintenance.
