Traditional Home Health Care Pediatric Division is seeking a compassionate Licensed Practical Nurse (LPN) to join our home-based care team for pediatric and adult patients. We offer weekday and weekend shifts with full-time or part-time schedules, and a pay rate of $28-$32 per hour.

Join us for dedicated one-on-one patient care in the home setting, flexible scheduling, and the support of a team focused on restoring health and independence for patients in Nanticoke, PA and surrounding areas.

Qualifications

• LPN license must be current and in good standing.

Responsibilities

• One to One patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members
