Qualifications

• Licensed practical nurse with current LPN license.
• Experience in home health or pediatric/adult skilled nursing preferred.

Responsibilities

• One-to-one patient care in home or school settings.
• Implement and document the plan of care for quality and continuity.
• Administer medications, treatments and therapies as prescribed.
• Conduct patient assessments and coordinate care with team.
• Report changes in client condition to Clinical Manager/physician.
• Health teaching and training for family members.

Responsibilities

• One to One patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members

Available Nursing Shifts

• Morning, evening or overnight
• 8’s, 10’s, or 12’s (full shift work)
• PRN, Part-Time, or Full-Time
• No weekend or minimum requirements
