Traditional Home Health Care Pediatric Division in Lake Ariel, PA is seeking a compassionate Licensed Practical Nurse (LPN) to join our team and provide high-quality home health care to pediatric and adult patients in their homes. This role offers weekend and evening shifts with potential for split shifts and school-hour options.

Qualifications

• Current LPN license in good standing.
• Experience in home health or pediatrics preferred.
• Ability to provide one-on-one patient care in home/school settings.

Responsibilities

• One-to-one patient care in home/school environment.
• Implement and document the plan of care to ensure quality and continuity of care for patient.
• Administration of prescribed medication, treatments and therapies.
• Patient assessments; coordination of care.
• Reports any changes in the client condition to Clinical Manager and physician.
• Health promotion, teaching, and training of family members.

Job Title: Licensed Practical Nurse

Service Location: Lake Ariel, PA

Immediate Shift Opening: Saturday & Sunday 7am-11pm, shifts can be split, Monday, Thursday,& Friday 6pm-6am

Immediate Shift Opening: LPN or HHA: School hours 7am-4pm & 9pm-7am

Rate: $28-$32 per hour
