Job Title: Licensed Practical Nurse
Service Location:Auburn, PA
Immediate Shift Opening: 2 nights per week, 8:30pm-5:30am, would like Mondays
Temporary Need: Monday through Friday 5:30am-4:30pm
Rate:$28-$32 per hour

Responsibilities:

• One to One patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members
