Traditional Home Health Care Pediatric Division in Luzerne County seeks a compassionate Licensed Practical Nurse (LPN) to provide one-on-one in-home care for pediatric and adult patients. Flexible weekday and weekend shifts, with full-time and part-time options, are available near Nanticoke, Plymouth, Hazleton, and West Hazleton.

We offer $28-$32 per hour with weekly pay, paid time off from day one, health/dental/vision benefits (where eligible), 401k with employer match, paid training, and 24/7

Qualifications

• LPN licensure or certification is required.
• Experience delivering in-home care to pediatric and adult patients preferred.
• Ability to document care plans and coordinate with clinical staff.

Responsibilities

• Provide one-to-one patient care in home or school settings.
• Implement and document the care plan to ensure quality and continuity of care.
• Administer prescribed medications and therapies.
• Assess patients and coordinate ongoing care.
• Report changes to Clinical Manager and physician.
• Educate and support family members on health maintenance.

Shift: Weekday and weekend shifts, full-time and part-time schedules available!

Rate: $28-$32 per hour

Immediate Need

• Nanticoke, PA: Weekends 7am-4pm
• Plymouth, PA: Monday - Friday 9am-3pm
• Hazleton, PA: Friday & Saturday 10pm-7am
• West Hazleton, PA: Thursday - Sunday 6am-6pm or 7am-7pm

Responsibilities

• One to One patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members

Available Nursing Shifts

• Morning, evening or overnight
• 8's, 10's, or 12's (full shift work)
• PRN, Part-Time, or Full-Time
• No weekend or minimum requirements
