Traditional Home Health Care Pediatric Division is seeking a compassionate Licensed Practical Nurse (LPN) to deliver one-on-one in-home nursing care for pediatric and adult patients. Flexible weekday, weekend, full-time and part-time shifts are available across multiple locations in PA.

Qualifications

• Must hold a valid LPN license.
• Experience in home health or pediatric care preferred.
• Comfort caring for pediatric and adult patients.
• Willingness to travel to assigned locations.

Responsibilities

• One to One patient care in home environments.
• Implement and document the plan of care for quality and continuity of care.
• Administer prescribed medications, treatments and therapies.
• Perform patient assessments and coordinate care with clinicians.
• Report changes in client condition to Clinical Manager and physician.
• Provide health promotion, teaching, and training to family members.

Job Title: Licensed Practical Nurse

Shift: Weekday and weekend shifts, full-time and part-time schedules available!

Rate: $28-$32 per hour

Immediate Need:

• Nanticoke, PA: Weekends 7am-4pm
• Plymouth, PA: Monday - Friday 9am-3pm
• Hazleton, PA: Friday & Saturday 10pm-7am
• West Hazleton, PA: Thursday - Sunday 6am-6pm or 7am-7pm
