Traditional Home Pediatric/Adult Skilled Division is seeking a licensed practical nurse to provide one-on-one home-based care in Hazle Township, PA. The Monday–Friday shift runs 9:00pm–6:00am, offering competitive pay with weekly deposits and comprehensive benefits.

Qualifications

• Must hold an active LPN license
• Excellent patient communication and empathy

Responsibilities

• Provide one-to-one patient care in home and school environments
• Implement and document plans of care to ensure quality and continuity
• Administer prescribed medications, treatments and therapies
• Perform patient assessments and coordinate care with clinicians
• Report changes in patient condition to clinical manager and physician
• Educate and support families on health maintenance

Rate: $28-$32 per hour
