Traditional Home Health Care Pediatric Division is seeking a compassionate Licensed Practical Nurse (LPN) to join our team in Bloomsburg, PA. We provide one-on-one care to pediatric and adult patients in the home and school settings, delivering medications, treatments and ongoing care planning with a focus on quality and continuity.

We offer flexible shifts (morning, evening, overnight), full-time or part-time, with competitive pay ($28–$32 per hour) and weekly pay with direct deposit.

Responsibilities

• One to One patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members
