Job summary

Traditional Home Health Care Pediatric Division in Luzerne County is seeking a licensed practical nurse (LPN) to provide compassionate, one-to-one care in home and school settings. We offer weekday/weekend shifts, flexible full-time and part-time schedules, and competitive hourly rates.

You will administer medications, implement care plans, perform patient assessments, and coordinate with clinical managers to ensure high-quality, safe care for pediatric and adult patients in clients' homes.

Qualifications

• LPN license required.
• Experience in home health care or pediatric care is a plus.

Responsibilities

• One-to-one patient care in home/school environments.
• Implement and document the plan of care to ensure quality and continuity of care.
• Administration of prescribed medications, treatments and therapies.
• Patient assessments; coordination of care.
• Report changes in client condition to Clinical Manager and physician.

Shift:Weekday and weekend shifts, full-time and part-time schedules available!
Rate:$28-$32 per hour
Immediate Need:
Nanticoke, PA: Weekends 7am-4pm
Plymouth, PA: Monday - Friday 9am-3pm
Hazleton, PA: Friday & Saturday 10pm-7am
West Hazleton, PA: Thursday - Sunday 6am-6pm or 7am-7pm
