Job summary

Sweetwater Care in California is seeking an LVN nurse manager to oversee nursing care within scope of practice, ensuring patient needs are met according to standards and physician orders.

You will lead in infection control, participate in quality improvement, and coordinate with physicians and other departments to ensure high-quality patient care in a long-term care setting.

Qualifications

• Licensed LVN in California; compact license if working in a Compact state.
• 1 year of nursing management experience preferred.
• Long-term care experience; PointClickCare charting experience preferred.

Responsibilities

• Demonstrates knowledge and appropriate use of the Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Uses the Quality Life Manual to identify, place, and monitor patients in appropriate programs.
• Notifies departments of patient changes (room changes, LOA, admits, discharges).
• Educates patients and families.
• Administers medications timely per facility policy.
• Administers treatments timely per facility policy.
• Receives, transcribes, and carries out physician orders as allowed by the Nurse Practice Act.
• R rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
