Job summary

Elara Caring is seeking a Licensed Vocational / Practical Nurse to deliver compassionate, high-quality home health care. You will follow patient care plans, document medications, and coordinate with the care team to ensure positive patient outcomes.

The role requires an active LVN/LPN license, nursing education, and at least 1 year of clinical experience, with travel between patient homes. A valid driver’s license or reliable transportation is required.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1 year of experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred.

Responsibilities
