Job summary

Elara Caring in Wisconsin is seeking a Licensed Vocational Nurse (LVN) to join our Skilled Home Health team. You will follow each patient’s care plan, work with the RN Case Manager, and provide compassionate, high-quality care to patients in their homes.

Requirements include current LVN/LPN license, graduation from an accredited nursing program, at least 1 year of clinical experience (Med/Surg or geriatrics preferred), and a valid driver’s license.

Qualifications

• Current LVN/LPN license in good standing.
• Graduate of an accredited nursing program (certificate/diploma/ADN).
• 1 year of clinical care experience; Med/Surg or geriatrics preferred.
• Experience in hospice or adult home health is a plus.

Responsibilities

• Follow each patient's care plan and physician orders.
• Monitor patient status and report changes to the care team.
• Educate patients and caregivers on self-care techniques.
• Collaborate with the RN Case Manager to update care plans.
• Document medications and update patient profiles per guidelines.
• Communicate with physicians, patients, and families to coordinate care.
• Help prevent avoidable hospitalizations through proactive care.

As aLicensed Vocational / Practical Nurseyou’llcontribute to our success in the following ways:

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting*** Ideally Med/Surg or Critical care***
• Experience in a hospice or adult/geriatric home health environment is preferred
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
