Job summary

Elara Caring in Cincinnati, OH is seeking a Licensed Vocational / Practical Nurse to deliver high-quality home health care under physician orders and care plans. You will monitor patient progress, document medications, collaborate with the RN Case Manager, and educate patients and families on self-care techniques.

A state LVN/LPN license and at least 1 year of clinical experience are required, with hospice/home health experience preferred.

• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Experience in a hospice or home health environment is preferred
