Job summary

Elara Caring is seeking a Licensed Vocational/Practical Nurse to join our home health team. You will follow individualized care plans, monitor patient progress, and communicate with the care team to ensure coordinated care across patient homes.

The role requires an active LPN/LVN license, a nursing credential from an accredited program, and at least 1 year of clinical experience. Travel between patient homes is required, with mileage reimbursement and comprehensive benefits offered.

Qualifications

• Graduate of an accredited certificate, diploma, or associate nursing program.
• 1 year of clinical care experience.

Responsibilities

• Follow each patient’s individualized care plan and physician orders.
• Monitor patient progress and report changes to the care team.
• Educate patients and families on self-care techniques.
• Coordinate care with RN Case Manager and update plans as needed.
• Document medications and update medication profiles per guidelines.
• Communicate with physicians, patients, families, and care team for coordinated care.
• Help prevent avoidable hospitalizations and emergency visits through proactive care.
• Deliver compassionate, high-quality care for positive patient outcomes.

As a Licensed Vocational / Practical Nurse you’llcontribute to our success in the following ways:

• Follow each patient’s individualized care plan and physician orders.

• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
