Job summary

Elara Caring is seeking a Licensed Vocational / Practical Nurse to join our Skilled Home Health team in Farmington, MO. You will follow care plans, monitor patient progress, and educate patients and families in self-care to support health and independence.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1 year of experience in a clinical care setting, ideally Med/Surg or Critical care.
• Experience in a hospice or adult/geriatric home health environment preferred.
• Must possess a valid driver's license or reliable transportation to travel between patient homes.
• Ability to frequently lift up to 50 pounds.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

Education

Graduate of an accredited Certificate, Diploma, or Associate School of Nursing

As a Licensed Vocational / Practical Nurse you’ll contribute to our success in the following ways:

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting *** Ideally Med/Surg or Critical care***
• Experience in a hospice or adult/geriatric home health environment is preferred
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
