Job summary

Elara Caring is seeking a licensed LVN/LPN in Texas to join our home health team. You will follow care plans, monitor patient progress, and collaborate with the RN Case Manager to adapt care as patient needs change.

Qualifications include an active LVN/LPN license, 1 year of clinical care experience, and the ability to lift up to 50 pounds. A valid driver's license or reliable transportation is required to travel between patient homes.

Qualifications

• Graduate of an accredited nursing program (certificate, diploma, or associate degree).
• Current State LVN/LPN license in good standing.
• Minimum 1 year of clinical care experience in a healthcare setting.
• Ability to frequently lift, push, pull, and transfer patients up to 50 lb.
• Must have a valid driver’s license or reliable transportation to travel between patient homes.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes to the care team.
• Educate patients and caregivers on self-care techniques.
• Collaborate with RN Case Manager to update care plans as needs change.
• Document medication regimens and update medication profiles per guidelines.
• Communicate with physicians, patients, families, and care team for coordinated care.
• Help prevent unnecessary hospitalizations through proactive care.
• Deliver compassionate, high-quality care to support positive outcomes.

Education

Graduate of a nursing program (Certificate/Diploma/Associate)

LVN/LPN license

What is Required

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
