Job summary

Elara Caring in Baton Rouge, LA is seeking a Licensed Vocational / Practical Nurse to join our home health team. You will follow patient care plans, monitor progress, and support families in learning self-care techniques to promote safe, effective care at home.

Qualifications include current LVN/LPN license, nursing program diploma/associate, at least 1 year in a clinical setting, ability to lift up to 50 pounds, and reliable transportation to travel between patient homes.

Qualifications

• Current State License as LVN/LPN in Louisiana.
• Graduate of an accredited Nursing program (Certificate, Diploma, or Associate).
• 1 year of experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Must have a valid driver's license or reliable transportation to travel between patient homes.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
