Job summary

Elara Caring in Ottumwa, IA is seeking a Licensed Vocational / Practical Nurse to join our home health team. You will follow care plans, monitor patient progress, and document medications according to agency guidelines.

The role emphasizes teamwork with the RN Case Manager, coordination with physicians and families, and delivering compassionate, high-quality care to support positive patient outcomes. A state LVN/LPN license and at least 1 year of clinical experience are required.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1 year of experience in a clinical care setting.
• Ability to frequently lift up to 50 pounds including positioning or transferring patients.

• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Experience in a hospice or home health environment is preferred
