Job summary

CVS Health Corporation is seeking a licensed Behavioral Health Clinical Case Manager to provide remote, full-time case management focused on mental health and substance use disorders. You will conduct assessments, develop care plans, coordinate with providers, and intervene during crises to improve member outcomes.

The role requires 3+ years of post-master's experience, a valid state license, and proficient use of EHR in a virtual environment.

Qualifications

• Master's degree in Behavioral Health, Social Work, Counseling, Psychology, or a related field.
• Active, unrestricted independent behavioral health license in your state of residence (e.g., LCSW, LISW, LMFT, LPC, LCPC, LPCC, LMHC)
• 3+ years of post-master's direct clinical experience in hospital, ambulatory, outpatient, or similar settings
• 3+ years of experience treating mental health and substance use disorders
• 3+ years of experience utilizing Motivational Interviewing techniques
• Case Management experience
• Ability to talk on the phone and document in clinical systems
• Strong technical proficiency and comfort working in a virtual environment

Responsibilities

• Conduct comprehensive behavioral health assessments using clinical tools, data analysis, and benefit plan reviews
• Develop individualized care plans addressing behavioral and physical health needs
• Coordinate care with providers, community organizations, and support services
• Apply advanced clinical judgment to identify and mitigate risk factors impacting outcomes
• Perform crisis intervention and ensure appropriate referrals and follow-up
• Utilize Motivational Interviewing to engage members and promote behavior change
• Coordinate psychosocial wraparound services and connect with community resources
• Support cost-effective care through proactive planning and resource utilization
